Question: What is Onda lifting and how does it work?
Onda lifting is a non-surgical treatment using a device made by the Italian company DEKA that delivers 2.45 GHz microwaves (Coolwaves) beneath the skin. The manufacturer explains that this energy acts on the subcutaneous fat layer and dermal collagen, and it is used on both the face and body. Research evidence is still small in scale and reactions vary from person to person, so suitability and the recovery process are checked during the consultation.

Onda lifting uses a handpiece that emits 2.45 GHz microwaves together with an integrated cooling system.
The device is made by DEKA, based in Calenzano near Florence, Italy, and the manufacturer calls this microwave technology Coolwaves. Published papers also describe it as using a 2.45 GHz microwave handpiece with integrated cooling.
Whereas HIFU (ultrasound) lifting focuses ultrasound energy on a point and radiofrequency (RF) lifting uses electrical current, Onda uses a different type of energy: microwaves. This only means the type of energy is different, not that one is superior, and the choice depends on skin condition.

The manufacturer explains that Coolwaves selectively target subcutaneous fat cells and stimulate contraction and production of dermal collagen.
DEKA explains that Coolwaves act on fat cell membranes, also affect the connective tissue between fat lobules in cellulite, immediately contract dermal collagen fibers and stimulate new collagen production. The manufacturer also explains that, unlike radiofrequency, which mainly disperses energy in the superficial layers, Coolwaves are preferentially absorbed by fat tissue.
This is the manufacturer's description, and actual reactions differ from person to person depending on skin thickness, the amount of fat and skin firmness.
The manufacturer explains that three handpieces, Deep, Shallow and Pocket, deliver energy at different depths.
According to the manufacturer, Deep is used for localized fat over large areas, Shallow for cellulite, skin laxity and the area under the chin, and Pocket for facial contouring and tightening. The manufacturer explains that 5°C contact cooling protects the surface of the skin during treatment.

Research on Onda is still small in scale, and an international advisory board consensus also states that evaluation of long-term effects and safety is lacking.
In a small study in which 19 adults with abdominal fat had 3 treatments 4 weeks apart, abdominal circumference decreased by an average of 3.80 cm after 3 months, and no severe pain during treatment was reported. A retrospective study of 171 East Asian patients reported that the microwave-based device showed faster early improvement at 1 month, while HIFU showed greater sustained improvement at 3 months.
